Speedway United Methodist Church (SUMC) would like to use photographs and/or videos of individuals in all our ministries periodically to update our website and social media pages (e.g., Facebook, etc.), as well as promotional purposes. This includes photos of children in our SUMC Child Care ministry during the school day, at summer camp, or at special events (e.g., parades, field trips, etc.). It also includes individuals (both members and visitors) in our other ministries such as our children’s ministry, youth ministry, and other ministry and fellowship activities (including parades, concerts, community days, etc.). To get access to any photos used, follow us on Facebook and/or sign up to be added to our SUMC newsletter sent out every other Friday which often includes photos from these events. This media release gives SUMC permission for images or videos of you or your child to be used in ALL SUMC events, activities, and ministries, including but not limited to the following: Child Care Ministry (during the school day and all events outside of school day), Children’s Ministry including Kids Worship Zone (Sunday Mornings) or other worship activities, Youth Group, and Fellowship Activities at the Church and in the community.

I understand that myself or my child(ren) whose name(s) I have written below may be photographed at any events or activities indicated above. With my signature below, I grant permission for myself or my child(ren) to be photographed, or their images recorded for print or electronic use in promoting SUMC services and ministries. I understand that it is MY responsibility to update this form in the event that I no longer wish to authorize the above uses. If my child is enrolled in the SUMC Child Care Ministry, I agree that this form will remain in effect during the term of my child’s enrollment. I understand that there will be no payment for my or my child’s participation in this release.
